Wedding guest forms for the details your RSVP doesn't cover
PlanMe's guest forms collect what your RSVP doesn't: dietary needs, flight times for airport pickups or mailing addresses. Start from a template or a blank form, share one link, and guests type their answers without an account. Every response lands in one table you can export as CSV. Free during beta.

What's included
Four ready templates
Save the Date, Dietary Preferences, Airport Transportation and Contact Information, each with suggested questions.
Import questions
Bring questions in from CSV or Excel, a Word document, or a Google Forms link.
One shareable link
Guests open the link and answer. There's no account to make and no password to remember.
A closing date
Set a date and time after which the form stops taking responses.
CSV export
Download every response as a CSV, with one column per question and the time it was submitted.
Team access
Forms has its own area in team roles, so you decide who on your team sees the answers.
How guest forms work
RSVPs tell you who's coming to which function. Forms gather the rest of what you need to know in one place. For yes or no replies per function, see RSVP for multiple events.
Questions and answers
Every question takes a written answer, so guests can reply in their own words: a flight number, an allergy, a full mailing address. That includes questions imported from Google Forms: the question text comes across, but multiple-choice and checkbox options don't, so those become written-answer questions too. A guest has to answer at least one question to submit and can leave the rest blank.
You can show an image beside the form with a line of text over it, such as your names and dates, or switch the image off in the form's settings.
Responses
Each response shows when it came in and the answer to every question. You can also add a response yourself, say for an aunt who read you her details over the phone, and edit or delete any response.
Responses aren't matched to your guest list, so add a name question if you need to know who answered. The Dietary Preferences and Contact Information templates already start with one.
Sharing the questions first
Before the form goes out, export the questions as a CSV or a Word document so family can look them over. Once you have what you need, set a closing date. After that, anyone who opens the link is told the form is closed and asked to contact the host.
Guest forms questions
What should I ask in a wedding guest questionnaire?
Ask only what your RSVP doesn't already cover: each guest's name, a phone number and mailing address, dietary needs and allergies, accessibility needs, and arrival details such as the airport and landing time if you're arranging pickups. PlanMe's templates start you with these questions.
Can I make a wedding guest questionnaire in PlanMe?
Yes. Create a form from a template or a blank page, write your questions and share the form's link with guests. Their answers appear in a table you can export as CSV.
Do guests need an account to fill out a form?
No. Guests open the form link and type their answers. They don't sign in or create a password.
Can I collect airport pickup details from wedding guests?
Yes. The Airport Transportation template asks how many people are traveling, a contact name and number, the arrival airport, the landing time and how many bags the group has. Edit those questions to fit your weekend.
Can I import questions from Google Forms?
Yes. Paste the link to a public Google Form to bring in its question text. Multiple-choice and checkbox options aren't copied, so every imported question takes a written answer. You can also import questions from a CSV or Excel file or a Word document.
Can I stop a form from taking responses?
Yes. Set a closing date and time. After it passes, guests who open the link see that the form is closed and are asked to contact the host.
